About Med Care Outreach

Med Care Outreach is a nonprofit organization dedicated to helping individuals and families better understand and navigate the healthcare system. Our mission is to reduce confusion, delays, and barriers to care by providing health education, care coordination support, and healthcare navigation assistance to underserved and medically complex populations.

We work alongside individuals and caregivers to support understanding of care options, provider communication, and ongoing health needs, particularly for those managing chronic conditions. Our approach emphasizes education, organization, and continuity of care so individuals and families can make informed decisions throughout their healthcare journey.

Our Work in Care Coordination and Chronic Care Support

As part of our educational mission, Med Care Outreach provides information and support related to care coordination and chronic care support. Care coordination focuses on helping individuals understand how different providers, services, and care plans work together across the healthcare system.

For Medicare beneficiaries living with multiple chronic conditions, this may include education about Chronic Care Management (CCM), a Medicare-approved care coordination approach that supports ongoing communication, care planning, and follow-up in collaboration with a patient’s physician.

Our role is to help individuals and caregivers understand how care coordination works, what questions to ask, and how to navigate available resources. We do not replace a physician’s care and do not make clinical decisions.

Our Experience and Community Focus

Our team includes licensed healthcare professionals with decades of combined experience in clinical care, care coordination, and health system navigation. We have worked with hospitals, physician practices, and community organizations to support individuals who face challenges accessing and understanding healthcare services.

Through education and outreach, we aim to promote equity, clarity, and continuity of care for communities that have historically experienced gaps in access and information.
